Eye Hospital : Process addresses discharge of all patients including Medico-legal cases and patients leaving against medical advice

 

..............Hospital has following discharge process for all the patients including MLC and DAMA/LAMA.

SOP for Discharge Process including MLC and LAMA
Sr. No.	Process Flow
1	Doctor examines the patient & summons discharge when patient is fit.
2	The Optometrist/nurse assistant explains the medications & post-operative care including do’s & don’ts
3	Discharge paper, do’s & don’ts postoperative care sheet is handed over to the patient. Patient/Relative acknowledgement is taken on the copy of discharge card.
4	Patient is instructed to report immediately in case of  redness, pain &/ or blurred vision post-operatively
5	Follow up appointment is given
6	Payment process is completed
7	In case of MLC, copies of all the papers are given to the patient & originals are given to the police & acknowledgement is obtained.
8	In case of death, the body is handed over to the police for post mortem.
9	In case of LAMA or DAMA, patient is informed about the risk of stoppage of treatment & consent is taken

b)     A discharge summary as detailed below is given to all the patients including DAMA/LAMA.

c)     The discharge summary contains the reasons for admission, all the findings, diagnosis, procedure performed, treatment given and the patient’s condition at the time of discharge.

d)     Discharge summary contains follow up advice, medication and other instructions in English, and is also explained to the patient/relatives by the Optometrist/counsellor.

e)     Discharge summary incorporates instructions about when and how to obtain urgent care.

f)         In case of death, the summary of the case includes the cause of death.
